Y-Be-Fit
A staff of faculty and students trained in fitness and health promotion manages the Y-BE-FIT program. While the Wellness Program focuses on group events, Y-Be-Fit is a personalized, one-on-one assessment and counseling program to specifically identify and target your health needs. Disease prevention and lifestyle modification are the two main areas of focus throughout the individualized 4-month program. During your first, second, third, and subsequent (follow-up) visits, you will be involved with the following:

Welcome to the Y-Be-Fit Comprehensive Wellness Program! We want to congratulate you for taking interest in you own health and well being by participating in this program. We know this step will help you in your pursuit of optimal health. This following lists materials and explains procedures necessary to participate in the Y-Be-Fit program.
You will receive the following material during your first visit:
Preliminary Medical Screening Questionnaire If you answered 'yes' to one or more question (s) on the questionnaire, you may be given a Physician's Release form to take to your doctor for his/her release before proceeding with the physical fitness testing portion of the program.
Informed Consent
For your personal safety and rights, an informed consent must be signed prior to fitness testing. Please read this form carefully and completely. Your signature and a witness's signature are required on this form.
Health Risk & Lifestyle Appraisal
This booklet contains questions about your general health and lifestyle. All of the answers to the questions should be filled in on the bubble sheet provided (except for those questions labeled men only or women only) using a #2 pencil. Also complete the appropriate bubbles with your name and other personal information as indicated.
3-Day Food Intake Diary
The 3-Day Food Intake Diary is used to evaluate your diet. This is a cumulative record of three consecutive days (two weekdays and one weekend day) of food intake. The diary is entered into a nutrition analysis program in the computer and analyzed for its calorie level and nutritional content.
FirstMed Blood Worksheet
The blood worksheet will be used to evaluate your blood cholesterol (including HDL, LDL, and VLDL) and triglycerides. Please print your name, gender, birth date, and phone number in the spaces provided at the top of the worksheet. Take this with you to FirstMed located at 800 N. 190 W. in Orem. To ensure that the results will be accurate, you must fast for 12 hours prior to having your blood drawn. This means no food or drink (water is OK) 12 hours before the blood is drawn.

Complete Assessment (second appointment)
The initial assessment at Y-BE-FIT is comprehensive and includes:
- Blood Pressure
- Blood Lipid Profile (total cholesterol, HDL, LDL, and triglycerides)
- Nutritional Analysis
- Body Composition/Percent Body Fat (Bod Pod)
- Cardiorespiratory Fitness (treadmill test)
- Muscular Strength (bench/leg press)
- Flexibility
- Back Health
- General Lifestyle (health risk appraisal)
- Results, Education, and Development of a Personalized Plan (third appointment)
Test Results, Education, and Prescription (third appointment)
After the initial screening, participants are given a detailed guidebook containing helpful information about their test results. A staff member reviews the test results and personalized book with the individual, giving an explanation about what the test results mean and identifying areas where health improvement can be made.
- Follow-up counseling (up to three months)
Follow-Up Counseling (up to three months)
Participation in the Y-BE-FIT program includes up to three months of follow-up for those individuals who desire additional counseling and monitoring while working to meet important health and fitness goals. The follow-up program is structured around the individual needs of the participant and focuses on topics such as weight control, aerobic conditioning, strength training, flexibility, cholesterol and blood pressure reduction, nutrition, stress management, etc. The follow-up program also includes a reassessment (at three months) to monitor progress.
Program Cost
If you are a DMBA insured employee (Managed/Network or Basic Care), 90% of assessment and follow up costs will be covered. This includes a comprehensive assessment, education, a personalized book, and a three-month follow-up for a co-pay of only $20.00. Additional three-month follow-up sessions will cost $40.00 each.
If you are a non DMBA insured employee or spouse interested in a complete assessment and three-month follow-up, the cost is $110.00. Additional three-month sessions will cost $40.00 each.
If you are a BYU student interested in a complete assessment and three-month follow-up, the cost is $70.00. Additional three-month sessions will cost $30.00 each.

- Glucose Test - $5.00
- Nutritional Evaluation - $15.00
- Treadmill Test - $15.00
- Skinfold (% body fat) - $1.00
- Osteoporosis Screening - Full body $130.00, Hip Scan $65.00, Spine Scan $65.00
*Hospital grade, full body osteoporosis screening by DEXA is now available for DMBA-insured postmenopausal women (45 or older), only a $13.00 co-pay for one scan (please provide written proof DMBA will cover the scan).
